US Agent Desk charges one yearly price per company: Entry $990, Standard $1,490 or Plus $1,950. Each plan covers one establishment; extra facilities are add-ons. Plans renew yearly and can be cancelled any time. FDA's own fees, such as the device establishment fee, are paid by your company directly to FDA.
